    Impact of TV Violence on Aggression of Youth; A Comparative Analysis of Male and Female Students of Lahore

    This study focuses on impact of TV violence on aggression young viewers of Lahore. For this study 500 (Male, 250 and Female 250) students are taken as a sample size from different universities of Lahore. Data is collected through stratified and convenience sampling technique from the targeted population. Major results indicate the significant difference in exposure to TV violence and aggression of male and female students. Major results of statistical tests male student exposure and attitude to TV violence has significant impact on aggression but exposure of female student has not significant impact on aggression whereas attitude to TV violence has significant impact on aggression. Further, there is also need to set some sort of filters on media contents which are presenting violence

    Analysis of localized solutions in coupled Gross-Pitavskii equations

    Bose-Einstein condensates (BECs) have been one of the most active areas of research since their experimental birth in 1995. The complicated nature of the experiments on BECs suggests to observe them in reduced dimensions. The dependence of the collective excitations of the systems on the spatial degrees of freedom allows the study in lower dimensions. In this thesis, we first study two effectively one-dimensional parallel linearly coupled BECs in the presence of external potentials. The system is modelled by linearly coupled Gross-Pitaevskii (GP) equations. In particular, we discuss the dark solitary waves and the grey-soliton-like solutions representing analogues of superconducting Josephson fluxons which we refer to as the fluxon analogue (FA) solutions. We analyze the existence, stability and time dynamics of FA solutions and coupled dark solitons in the presence of a harmonic trap. We observe that the presence of the harmonic trap destabilizes the FA solutions. However, stabilization is possible by controlling the effective linear coupling between the condensates. We also derive theoretical approximations based on variational formulations to study the dynamics of the solutions semi-analytically. We then study multiple FA solutions and coupled dark solitons in the same settings. We examine the effects of trapping strength on the existence and stability of the localized solutions. We also consider the interactions of multiple FA solutions as well as coupled dark solitons. In addition, we determine the oscillation frequencies of the prototypical structures of two and three FA solutions using a variational approach. Finally, we consider two effectively two-dimensional parallel coupled BECs enclosed in a double well potential. The system is modelled by two GP equations coupled by linear and nonlinear cross-phase-modulations. We study a large set of radially symmetric nonlinear solutions of the system in the focusing and defocusing cases. The relevant three principal branches, i.e. the ground state and the first two excited states, are continued as a function of either linear or nonlinear couplings. We investigate the linear stability and time evolution of these solutions in the absence and presence of a topological charge. We notice that only the chargeless or charged ground states can be stabilized by adjusting the linear or nonlinear coupling between the condensates

    Impact of unplanned excision on prognosis of patients with extremity soft tissue sarcoma

    Unplanned excision of soft tissue sarcomas (STSs) outside comprehensive tumor management centers necessitates the need for wide reexcision to achieve adequate margins. We retrospectively reviewed medical records of 135 patients with STS operated at our hospital with the goal of examining outcomes, in terms of local recurrence (LR) and metastasis rate (MR), of reexcision following unplanned excision of STS and comparing results with those of first-time planned surgery. Eighty-four patients had their first-time surgery and 51 patients had come to us following unplanned excision at prereferral hospital. Mean age of all patients was 41.8 ± 21.9 years. The LR and MR was 14.3% and 8.3%, respectively, in patients undergoing first resection, whereas it was 21.4% and 13.7%, respectively, in patients undergoing revision surgery. Average duration from previous unplanned excision was 8 months. Twelve patients were referred immediately after excised specimen revealed STS, while 39 patients presented after evident local recurrence. Wide reexcision was attempted in 48 patients while three patients need amputation. Adjuvant radiotherapy was administered in all patients undergoing limb-sparing surgery. Ten patients needed adjuvant chemotherapy.We conclude that wide reexcision of STS has poorer outcomes compared to planned excision. Therefore, patients with soft tissue masses should be managed by multidisciplinary oncology team at specialized cancer centers

    Managing soft tissue sarcomas in a developing country: Are prognostic factors similar to those of developed world?

    Background: Managing soft tissue sarcomas (STS) in a developing country with limited financial resources and a poor health referral system is a challenge. Presenting late, these extremity STS are prone to recurrence despite apparently complete resection. This study aimed to explore and compare the impact of clinico-pathological factors on recurrence and survival in Pakistan with the corresponding figures quoted from the developed world. Methods: An institutional review was performed on all patients with primary STS of the extremities operated on between 1994 and 2008. The prognostic influence of clinical, pathologic, and treatment variables on local recurrence free survival (LRFS), metastasis free survival (MFS) and overall survival (OS) were analyzed by univariate and multivariate Cox regression analysis and Kaplan Meier survival curves. Results: A total of 84 patients with a mean age of 41.8 ± 21.9 years were included in the study. The local recurrence rate was 14.3% after a median of 6 (mean 7.4) months. Metastases occurred in 7 patients (8.3%) and 65 patients were alive without evidence of disease after a mean follow-up of 52.6 ± 39.8 months. Tumor size \u3e 5 cm, grade 3 tumors and margin \u3c 10 mm significantly increased local recurrence rates. A margin ≥ 10 mm and age \u3c 45 years significantly enhanced cumulative survival. Significant multivariate risk factors for metastases were margin \u3c 10 mm and tumor grade G3. Conclusions: Despite a poor health referral system in our country, our results are no different from those reported from the developed world.     Vulnerability of Environmental Resources in Indus Basin after the Development of Irrigation System

    The climatic and topographic characteristics of Indus Basin provided an excellent condition for the development of irrigation system. Archaeological remains of Harappa and Mohenjo-Daro indicated that several canals were constructed in this region. The Indus River System (IRS) was developed into a complex network of canals, and 74% of its water was utilized for irrigation after Indus Water Treaty. After 1947, Indus irrigation network was extended, and cropland area was increased from 8.5 to 18.2 MH in Pakistan and 2.02 to 8.5 MH in India. Construction of dams, barrages, and canals to divert the maximum river water for irrigation resulted in drying up the natural pathways of the rivers, except during monsoon season. The aquifer in the irrigated areas became high and created problems of waterlogging and salinity, but due to extensive groundwater extraction, water table near urban centers is lowered now. Water quality was degraded due to addition of fertilizers, pesticides, chemicals, municipal sewage, and industrial effluents. Due to climate change, the glaciers in the upper catchment areas are continuously retreating and the frequency of floods and droughts is increasing. The objective of this chapter is to provide a comprehensive review of irrigation system developments in Indus Basin and its implications on environmental resources

    Heart valve surgery: EuroSCORE vs. EuroSCORE II vs. Society of Thoracic Surgeons score

    Background: This is a validation study comparing the European System for Cardiac Operative Risk Evaluation (EuroSCORE) II with the previous additive (AES) and logistic EuroSCORE (LES) and the Society of Thoracic Surgeons\u27 (STS) risk prediction algorithm, for patients undergoing valve replacement with or without bypass in Pakistan.PATIENTS AND Methods: Clinical data of 576 patients undergoing valve replacement surgery between 2006 and 2013 were retrospectively collected and individual expected risks of death were calculated by all four risk prediction algorithms. Performance of these risk algorithms was evaluated in terms of discrimination and calibration.Results: There were 28 deaths (4.8%) among 576 patients, which was lower than the predicted mortality of 5.16%, 6.96% and 4.94% by AES, LES and EuroSCORE II but was higher than 2.13% predicted by STS scoring system. For single and double valve replacement procedures, EuroSCORE II was the best predictor of mortality with highest Hosmer and Lemmeshow test (H-L) p value (0.346 to 0.689) and area under the receiver operating characteristic (ROC) curve (0.637 to 0.898). For valve plus concomitant coronary artery bypass grafting (CABG) patients actual mortality was 1.88%. STS calculator came out to be the best predictor of mortality for this subgroup with H-L p value (0.480 to 0.884) and ROC (0.657 to 0.775).CONCLUSIONS: For Pakistani population EuroSCORE II is an accurate predictor for individual operative risk in patients undergoing isolated valve surgery, whereas STS performs better in the valve plus CABG group

    Evaluation of Antibiotics Pattern of Extended Spectrum Beta-Lactamase Producing Multi-Drug Resistant Pseudomonas aeruginosa

    Background: Pseudomonas aeruginosa (Ps. aeruginosa) is considered as an opportunistic pathogen and the leading cause of morbidity and mortality in immunocompromised individuals. Globally, approximately 10-15% of the nosocomial infections are caused by Ps. aeruginosa. The Ps. aeruginosa can acquire resistance against broad-spectrum antibiotics. According to recent studies increased mortality has been observed due to infection with extended-spectrum-beta-lactamase (ESBL) producing Ps. aeruginosa strains. This study was designed to determined antibiogram of ESBL producing multi-drug resistant Ps. aeruginosa in Khyber Pakhtunkhwa.Methods: The clinical confirmed Ps. aeruginosa samples were collected according to the standard protocol, at Khyber Teaching Hospital (KTH), Peshawar. All collected samples were sub- cultured on appropriate culture media. After isolation and identification, the antibiotics susceptibility testing was performed. The detection of ESBL was carried out by the double-disc diffusion method. Carbapenemase-producing bacteria was confirmed by the modified Hodge test. Descriptive analysis was performed for statistical analysis of collected data.Results: A total of one hundred and sixty-two (n=162) Ps. aeruginosa confirmed isolates were collected, in which 59.3% were male and 40.7% were from female patients. The percentages of ESBL and carbapenemase producing Ps. aeruginosa isolates were 5.5% and 23.5%, respectively. The multidrug resistance was observed against 27.2% isolates. Among tested antibiotics highest percentages of resistance was observed against ciprofloxacin (43%) and ceftazidime (39.5%). Conclusion: We observed highest level of drug resistance in Ps. aeruginosa clinical isolates against tested antibiotics and majority of the isolates were Multi-drug resistant (MDR).Keywords: Pseudomonas; Multi-Drug Resistant; Extended Spectrum Beta Lactamases; Antibiotics Susceptibilit

    The groundwater resources have always been explored for its utilization in domestic and agriculture purposes. In this study, Winder area in Balochistan which is along the coast of Offshore Indus Basin has been explored using electrical resistivity method for deeper understanding of hydrogeological and hydrogeophysical conditions. Schlumberger electrode arrays have been used to acquire vertical electrical sounding data for 23 different sites in this area. The field data were calibrated with theoretical curves during processing using software with graphical interface. The true resistivities, depths, thicknesses, aquifer attributes, soil type and groundwater quality has been extracted from the processed data. Pseudo-sections and geo-electrical lithology sections have been generated for lateral distribution of hydrogeological settings. Furthermore, Dar-zarrouk parameters calculations have been made and 3D surfaces generated at 30m and 60 m depth for transverse resistance (T), longitudinal conductance (S) and anisotropy (λ) plotted for resistivity distribution and coastal salinity intrusion analysis. The soundings results depict five layers mainly composed of silty sand, sandy clay, sand, and sandy gravel and the depth ranges from 0-110m. The interpretation illustrates that freshwater unconfined aquifer are present in the quaternary sand and sandy gravel deposits. The water table is at shallow depth along the river and deeper in northeast and away from Winder river. The resistivity ranges 29-1091 Ωm in layer-3 interpreted as saturated zone. The approximate range of water table is 19-30 m. The higher value of T greater than 10000 shows fresh water and low value 8200 for salinity intrusion in southwest toward sea.Izvori podzemne vode uvijek su se istraživali radi njihove upotrebe u kućne i poljoprivredne svrhe. U ovoj je studiji područje Windera u Beludžistanu, uz obalu priobalnoga bazena Inda, istraženo korištenjem metode električnoga otpora radi dubljega razumijevanja hidrogeoloških i hidrogeofizičkih uvjeta. Nizovi elektroda Schlumberger korišteni su za prikupljanje podataka o vertikalnome električnom sondiranju za 23 različita mjesta u ovome području. Podatci na terenu kalibrirani su teoretskim krivuljama tijekom obrade s pomoću softvera s grafičkim sučeljem. Iz obrađenih podataka izdvojena je stvarna otpornost, dubine, debljine, svojstva vodonosnika, vrsta tla i kvaliteta podzemne vode. Za bočnu raspodjelu hidrogeoloških postavki stvoreni su pseudopresjeci i geoelektrični litološki odsjeci. Nadalje, izrađeni su proračuni Dar-zarroukovih parametara i generirane su 3D površine na dubini od 30 m i 60 m za poprečni otpor (T), uzdužnu vodljivost (S) i anizotropiju (λ). Ti su parametri ucrtani za raspodjelu otpora i analizu upada slane obale. Rezultati sondiranja prikazuju četiri sloja uglavnom sastavljena od muljevitoga pijeska, pjeskovite gline, pijeska i pješčanoga šljunka, a dubina se kreće od 0 do 110 m. Tumačenje ilustrira da su slatkovodni vodonosnici prisutni u kvartarnim pjeskovitim i pjeskovitim naslagama šljunka. Vodeni je sloj na maloj dubini uz rijeku, a dublje na sjeveroistoku i dalje od rijeke Winder. Otpornost se kreće od 29 do 1091 Ωm u sloju 3, što se tumači kao zasićena zona. Približni je domet vodostaja 19 – 30 m. Viša vrijednost T veća od 10000 pokazuje slatku vodu i nisku vrijednost 8200 za prodiranje slanosti na jugozapadu prema moru

    Salvage of infected non-union of the tibia with an Ilizarov ring fixator.

    Abstract PURPOSE: To review outcomes of 24 patients who underwent Ilizarov ring fixation for infected nonunion of the tibia. METHODS: Medical records of 21 men and 3 women aged 13 to 74 (mean, 38) years who underwent Ilizarov ring fixation for infectednon-union of the tibia were reviewed. The mean bone defect was 3.3 (range, 2-5) cm. The mean time from injury to presentation was 11.9 (range, 1-36) months. The mean number of previous surgeries was 2 (range, 0-14). A local flap was used in 2 patients and a free flap was used in one patient. Nine of the patients underwent Ilizarov ring fixation without soft tissue and bony resection, as inadequate stability was the reason for non-union. Patients were assessed using the Association for the Study and Application of the Method of Ilizarov criteria. RESULTS: Patients were followed up for a mean of 11 (range, 8-46) months. Functional outcome was excellent in 8 patients, good in 12, fair in 2, and failure in one, whereas bone union outcome was excellent in 6 patients, good in 14, fair in one, and poor in 2. The mean time to union was 8 (range, 3-31) months. The mean external fixation index was 4.2 (range, 1.5-15.7) cm/month. Complications encountered were pin tract infection (n=5), re-fracture (n=2), soft tissue impingement by Ilizarov rings (n=2), recurrence of wound infection (n=1), mal-union (n=1), and mortality (n=1). CONCLUSION: Ilizarov ring fixation is a viable option for infected non-union of the tibia. Adequate assessment of bone union is crucial before removal of fixator to prevent re-fracture
